Teen to 21 Program
Our Teens to 21 transition to acute rehab program provides individualized PT, OT and ST to further prepare the patient for an Acute Rehab Setting before returning to home.
Program Features:
- PhD and Masters Level PT, OT and ST
- Nurse to Patient Ratios well above that required by state
- Daily AM rounding on all patients by Nursing and Respiratory Therapy Team
- Managed by team of Critical Care Intensivists
- 8-10 Respiratory Therapists on every shift
- Trach and Vent Weaning Unit
- PhD Psychologist and Social Services available to patient and family
- Multi Sensory Room
- Large enthusiastic Recreational Activities Team
- Masters Level Activities Director
- Child Life Specialist
- Two outdoor activity areas
- Age appropriate on campus activities such as theme parties, live music, pet therapy, music therapy, and art therapy
- Off campus outings to Mall, Beach, and Community Events
- On Campus School for continuing education
- Off Campus School attendance accompanied by Private Nurse
- Social Services coordination regarding all discharge planning
